Tests XD0038 is raised when document does not match explicit content-types on p:input
Test ab-contenttypes-004.xml is expected to fail with error code err:XD0038.

Added test to make sure p:pipe is no child of p:input. Making content-type statically in the last remaining tests.
Fix namespace declarations
Change test with document-properties to map{xs:QName, item()}
